We appreciate your interest! Application is made online. Below are the steps to guide you through the process.  
 

STEP 1 ADMISSION FORM - UPLOAD REQUIREMENTS:  
Fill out and complete the Admission Form, Submit the Requirements then click Submit

              For College, The Guidance Office will send an Acceptance Notification once your application and requirements are accepted.
              For SGS, The Deans Office will send an Acceptance Notification once your application and requirements are accepted.

Requirements for College: (New Students and Transferee)

  • Photocopy of SHS Report Card and Good Moral Certificate (New Students)
  • Photocopy of TOR and Honorable Dismissal (Transferee Students)
  • NSO/PSA Birth Certificate - 1 Photocopy
  • 2x2 Recent Pictures (red or blue background) - 2pcs
  • Baptismal & Confirmation Certificate (Catholic Students) - 1 photocopy
  • Marriage Contract (Married Students) - 1 photocopy
  • 1 Long Brown Envelope & Folder

Requirements for SGS: (New Students and Transferee)

  • Photocopy of TOR and Honorable Dismissal
  • NSO/PSA Birth Certificate - 2 Photocopies
  • Marriage Contract (Married Students) - 1 photocopy
  • 2x2 Recent Pictures (with collar & white background)
  • Professional Regulation Commission (PRC) - 1 photocopy
  • 1 Long Brown Envelope & Folder

 

STEP 2 ADMISSION FEE:  
Upon confirmation from the Guidance Office Pay the admission fee of 300 php via bank transfer or at the Accounting office.

Once payment is made, SCAN/SCREENSHOT your proof of payment then visit your School website Click Portal and Select Student login to your account using the username and password that was sent to you during admission.

Click TUITION on the side bar menu, Filter Academic Year and Semester to ADMISSION FEE, BROWSE and select image then click Upload Proof of Payment to submit.

Wait for the SMS/Email Confirmation for the schedule of your interview and screening.

STEP 3 & 4 INTERVIEW & SCREENING - MEDICAL/PHYSICAL EXAM:
Proceed for interview and screening with guidance counselor/ program heads
After the interview and screening, you will be informed by the Guidance Office if you are eligible for enrollment.

For the Medical/Physical Exam, proceed to School Clinic and get the Request for Laboratory and Xray

 

STEP 5 REGISTRAR OFFICE - SUBMISSION OF ORIGINAL REQUIREMENTS:

  • Original Copy of SHS Report Card/Honorable Dismissal/Transcript Record (For Transferee only)
  • 1 Photocopy of NSO/PSA Birth Certificate and Marriage Contract (for Married Student)
  • 2x2 Recent Pictures (red or blue background)
  • 1 Long Brown Envelope & Folder

 

STEP 6 ENLISTMENT & PAYMENT

Login your account > Click enlistment > Update your personal info > Answer the Health Survey > choose Payment Plan > Submit.

Pay upon enrollment fee directly to your School Cashier or via Bank Transfer within five 5 WORKING DAYS upon enlistment to avoid enlistment expiration. Once payment is made, SCAN/SCREENSHOT your proof of payment then visit your School website Click Portal and Select Student login to your account using the username and password that was sent to you during admission.

Click TUITION on the side bar menu, Filter Academic Year and Semester to TUITION FEE, BROWSE and select image then click Upload Proof of Payment to submit. You will receive an SMS/Email Confirmation of your enrollment status after your payment.

 

STEP 7 REGISTRATION FORM/ID PICTURE/UNIFORM MEASUREMENT

Go to Accounting Office area for registration form printing then proceed to Registrars Office to get the Certified true copy of registration form (for scholars only)

NOTE: Official Receipt can be claimed at the Cashier after (3) business days for Online Payment or Bank Payment.

  • Please present the original copy of the documents to the School Registrar on your campus visit.
  • Only files with the following file extensions are allowed : PDF, Excel, Docs, JPG, JPEG and PNG. Total file size must NOT be greater than 100.00 MB.
